Instructions for filling out the Membership Application Form

1. All sections of the form must be filled.
2. The information should preferably be filled on the computer, and the printout mailed to the address below. If filled out by hand, an ink pen must be used. No pencils.
3. Please staple pages 1 and 2 together before mailing.
4. Name must be written on both pages.
5. Please Print your name.
6. Please print both your addresses.
7. At least one of the addresses must be a regular (non-post office box address). The journal will not be mailed to a post office box address. Tick mark the address to which correspondence will be sent.
8. At least one phone number must be provided.
9. At least one email address must be provided.
10. Recommendation by a member of the ISPQS is required.
11. The form must be duly signed by the applicant.
12. The Indian Society for Prehistoric and Quaternary Studies reserves the right to refuse membership to any applicant without explanation.
13. Membership dues must be paid by demand draft. No personal checks or cash will be accepted.
14. Demand Draft is payable to:
